We are now offering Covid vaccinations for anyone ages 18 and up by appointment. Please follow the steps below.

*Please note we are offering the Primary series vaccine and the new Bivalent Booster vaccine. Both the Moderna and Pfizer Bivalent are available.

Step 1 

​

If preferred, you may print off and complete Sections A-E of the attached COVID-19 Informed Consent Form prior to your appointment. If you do not wish to complete the form prior, we have copies available to fill out when you arrive.

Step 2

​

Book an appointment for a vaccination time below, or come to one of our scheduled clinics.

Step 3

 

Bring your insurance card, COVID-19 Informed Consent Form (if filled out prior), and vaccine card with you to your appointment. *If you do not have a vaccine card, we can fill out a new card for you.

Step 4

 

Find a parking spot on the downtown block between the Galesville Library and the Treasure Chest. Call 608-468-6226 when you arrive and we will give you instructions. Please be patient and have all items listed in Step 3 ready. (You will be directed to the door on the north side of the building.)

Step 5

​

In the event that you experience any adverse symptoms following your vaccination, please call the pharmacy at 608-468-6226 so that we can report the incident through the Vaccine Adverse Event Reporting System (VAERS)
